1. All registrations will be conducted at the appropriate school site presenting documentation as requested on the LPSB Registration/Guardianship/ Attendance Zones form.
2. The following documents are required when the parent/legal guardian and child(ren) are residing with someone else and have no residency in any other state, parish or district and do not have 2 or more bills in the parent/legal guardian’s name: (Note: To be identified as homeless, a family must lack a fixed, regular and adequate nighttime residence due to loss of housing, economic hardship, or similar circumstances. Homeless is a temporary situation. Additionally, they must present the following:
• Affidavit of Residency (original document) purchased from a Notary (good for only one calendar school year) or a notarized written statement with the following information: â The child’s name.
â The parent/legal guardian’s name.
â The person’s name and address with whom they are residing.
â The notary seal/stamp or commission number is acceptable.
• Two proofs of address: (any two of the following most recent documents-within 30 days): Electric Bill, Telephone Bill, Water/Sewage, Cable Bill, Lease/Mortgage OR a lease can be used in place of both when utilities are included.
• Photo Identification (driver’s license/ state issued ID card, etc.) of the parent/guardian and the person whose name is on the bill.
• Birth Certificate, Social Security Card, and current immunization records of any child(ren) registering in LaSalle Parish School for the first time;
• Court Order and/or Custody Papers (if applicable)
• Withdrawal slip from the previous school (if applicable).
Students lacking the necessary documentation for registration may be enrolled on a temporary basis allowing them time to submit the necessary information. This includes, but not limited to the following circumstances:
• Parent/Guardian lacks one or both proof(s) of address;
• Parent/Child living with someone and parent has no bills in his/ her name;
• Parent/Guardian lacks the child’s birth certificate or immunization record.
3. Procedures for Temporary Enrollment without requested documentation: • A separate file must be maintained at the school site on students enrolling under temporary enrollment.
• School personnel will allow the parent/guardian an appropriate time frame (not to exceed fifteen (15) business days) to submit the required documentation.
• Once the parent brings in the documentation, the student file is removed from the temporary status and the registration process is complete.
• Once time has expired and documentation has not been submitted, school personnel should: â Contact the parent/guardian by phone.
â Send written correspondence by the student.
â If all efforts to obtain the necessary documents fail, school personnel will immediately submit a Home Visit Request Email to the Supervisor of Child Welfare Attendance for an investigation.
â The Supervisor will make a follow up contact with the parent/ guardian verifying residency and will submit findings to the school.
â Students shall remain enrolled at the school site until the Child Welfare and Attendance Office investigation is completed.
4. Students previously expelled or released from a juvenile facility and attempting to enroll in the LaSalle Parish School System from other school districts (out of parish/state, Parochial, Private, RSD and Charter schools) must be referred to the Office of Child Welfare and Attendance (CWA) for registration.
5. All custody issues should be referred to the Office of Child Welfare & Attendance unless the guardian presents appropriate documentation (custody awarded by a judge) at the school site.
Note: The LaSalle Parish School System DOES NOT accept provisional custody, custody by mandate, or power of attorney. Only the parent/ legal guardian of the child can enroll a child in school. If you have any questions, please contact your CWA supervisor at (318)992-2161.
